Required Documents:
 
  • Birth Certificate

  • Health Examination and Immunization Form (read requirements per grade below)

  • Dental Exam Report  (info, formwaiver) (read requirements per grade below)

  • Vision Exam Report (infoformwaiver) (read requirements per grade below)

  • Copies of Standardized Tests Reports (IOWA, ISAT, STANFORD, etc.)

  • Student Record Release Form

Health Requirements Per Grade


Pre-School

    • New Students Transferring From Out-of-State: 

      • A physical exam is required before October 15th of the year or within 30 days of enrollment.

      • A vision exam is required before October 15th or within 30 days of enrollment.

    • Other Students: 

      • An up-to-date physical exam is required for entry into preschool.

    • The physical exam must be dated by the doctor within one year prior to the date the student enters the school.

    • The “health history” portion of the physical exam must be filled out and signed by a parent in order for the physical to be valid.


Kindergarten

  • New Students Transferring From Out-of-State: 

    • A physical exam is required before October 15th of the year or within 30 days of enrollment.

    • A vision exam is required before October 15th or within 30 days of enrollment.

    • A dental exam is required before May 15th.  The exam must be dated within 18 months.

  • Other Students: 

    • An up-to-date physical exam is required for entry into Kindergarten.

    • A dental exam is required before May 15th.  The exam must be dated within 18 months.

    • A vision exam is required before October 15th or within 30 days of enrollment.

  • The physical exam must be dated by the doctor within one year prior to the date the student enters the school.

First Grade
